What info would i need to provide to get a new kindle?

My kindle, after about 3 months of use got black and white stripes on the sceen, i tried the oh-so-magical "turn of and hold for 20 seconds" no luck, what info would they need from me, to receive a new kindle in the mail? i heard there costermer support is really great, so iv'e heard....
